Firstly thank you for taking the time to help us. We want to improve our sites and to do this we need to know what you want from our sites.

A valuable part of the process is to understand your comments and react to them, so we would like to get your opinion on what we have been doing right, what we are doing wrong and what we could do in the future to improve.

To get to know you better we are also asking for a little bit of info about you and your internet usage to get a bit of a better idea about the kind of visitors we receive to the site.

This helps us to gauge what you want from the site, what you do and don't like in your general internet usage and thus hopefully we can make changes to the way the site is put together which are more in line with your comments.
